Online Forms powered by ZippSlip

The Schuylkill Haven Area School District will once again be using ZippSlip for Online First Day Forms.    We have consolidated the forms and created pre-populated online forms through the company ZippSlip.  You will have the ability to complete and approve partially-populated forms.  We have tried to upload as much data and demographics on each child as possible. 

Parents who have an email recorded in Power School for your child and receive School Messenger emails will receive an invitations directly from ZippSlip August 21, 2020..

  1. If you did not receive an email from ZippSlip, you will need to manually create an account with ZippSlip. Access to ZippSlip will be available after August 21, 2020.

    Go to: https://www.zippslip.com/Zippslip/PermissionManager/PA/district/SchuylkillHA

    Select:  Sign Up – next to the Don’t have a parent account yet?

                            You will need to log in:

                                            Valid email address

                                            Your child’s school student ID

                                            Your child’s date of birth

  2. School Student ID’s can be obtained in several places:
    1. Child’s last report card
    2. Power School Account
    3. Food Service information
    4. By contacting your child's building office

  3. Follow the instructions on the setup page. You will be required to enter an email address, a password and other information. On the next page, you will enter the student ID and Date of Birth for each your child(ren) attending the district. Once you login with your credentials, you will be asked to complete personal information and create a PIN (Personal Identification Number) to verify your acceptance of the forms.  The PIN number is your own creation and cannot be reset by the school district.  You will then enter your child(ren)’s information to associate to the account.

  4. The ZippSlip “My Dashboard” appears with the forms and your child(ren) packet(s).

  5. Select the first packet and complete the forms. Complete all necessary fields and those marked with red asterisks. You cannot advance to the next form without completing the red asterisk fields. Each form will require you to enter a PIN number for verification. At the completion of the form verifications, you will need to submit. You will receive a confirming email upon completion. Once submitted the building secretaries will have access to your forms.

  6. You can begin completing the forms as early as Friday, August 21, 2020  with the latest by September 3, 2020.

  7. You can access the forms through any device (laptop, desktop, tablet, cell phone).You may need to download the App from the Apple store or Google play.The App download is located at the Schuylkill Haven ZippSlip website: (https://www.zippslip.com/Zippslip/PermissionManager/PA/district/SchuylkillHA)

As the school year progresses, we will be utilizing the ZippSlip online forms website for various forms.  We will continue to send email notifications as the forms become available.  Please note, once you log in, your credentials will not change for future forms.
